Special Issue Information

Dear colleague,

End-stage liver disease and fulminant hepatic failure are the final stages of chronic and acute hepatic diseases, respectively. These clinical conditions are characterized by a poor 3-month survival with medical management other than liver transplant. Additionally, in the setting of hepatocellular carcinoma, liver transplantation represents, within defined criteria, the best therapeutic option.

In the last twenty years, the significant progress in the clinical management pre- and post-transplantation and the advancements in surgical techniques have guaranteed a progressive improvement of patient survival in the short and long term.

Data from the European Liver and Intestine Transplant Association show 1-year survival of 86% and 5-year survival of 74% for the liver transplants performed in European centers between 2010 and 2014.

The future challenges are to improve the referral to liver transplantation, to reduce the organ shortage in order to decrease the waiting-list mortality, and to improve the immunosuppressive strategies to minimize the side effects of long-term immunosuppressive therapy.
